Assignment 1: LASA 2—Final Report Portfolio Summary

In this LASA, you will create a final portfolio and summary report consisting of two parts.

Part I: Completed Forms

Your portfolio will begin with a title page and include the following documents and paper:

• The final copy of your completed Field Experience Log form (signed by your supervisor).
• Completed copy of your Field Experience Journal (in the form of a Microsoft Word document). Note: Use your Reflective Journal Entries from Modules 1, 2, and 4 to fill in your Field Experience Journal.
• Completed copy of your Field Experience Self-Evaluation form.
• Completed copy of your Field Experience Supervisor Evaluation form from your supervisor. Note: You must arrange to have your supervisor send the completed form to your instructor directly. You may not deliver the form to your instructor yourself; it must come directly from the supervisor.  

Part II: Summary of Field Experience Paper

Write a paper that synthesizes all of the above material and addresses the following:

1) A Detailed Description of Your Placement Activities

• Describe your placement site or agency and the tasks of the staff at your placement site.
• Explain your role at the placement site and how you fit into the team.
• Assess the services that are provided by the agency.
• Discuss any diversity-related issues you saw reflected in the population that you were serving.
• Identify one ethical issue (for example, confidentiality, informed consent, or issues of competence) that arose in your work site. Explain how you dealt with it. Cite and describe the relevant ethics codes from the American Psychological Association and the American Counseling Association.
• Identify at least three theoretical constructs from the various courses you have completed throughout your studies in the Bachelor of Arts, Psychology program. How have they influenced your observations of and behavior toward the population you are serving?
• Explain what you have learned about this area of psychology practice, how it fits into your future career goals, and if it is an area in which you would like to work after graduation and why. 

2) A Reflective Summary:

• Explain what you have learned about the population you served, the agency that serves them, and the broader social issues that affect them.
• Identify what you have learned about yourself (i.e., preconceptions, skills, experiences, and insights).
• Describe what you have learned about the influence of the community, society, agency, and the diversity needs of the population served.

Write an 8–10-page paper in Word format exclusive of a title page and reference page. In addition, be sure to include all of the above-completed forms. Apply APA standards to citation of sources and the formatting of your paper. Use the following file naming convention: LastnameFirstInitial_M5_A1.doc.

By Monday, January 26, 2015, deliver your assignment to the M5: Assignment 1 Dropbox.
